2026 Appalachian Health Leadership Forum Registration

The Appalachian Health Leadership Forum will be held on April 23 & 24, 2026 at Stonewall Resort & Conference Center in Roanoke, WV. For additional details about the Forum, including the agenda and room block information, please click here.

To register for the Forum, please complete the form below before registration closes on April 16, 2026. You will receive a confirmation email to the address provided upon receipt and processing of your registration form.

Full Conference Registration $400.00
Student Full Conference Registration Includes both Thursday  and Friday sessions and all meals and reception. Must be an active college/ university student to receive this rate. $200.00
One Day Only: Thursday, April 23, 2026 Includes breakfast, lunch and evening reception. $250.00
Student – One Day Only: Thursday, April 23, 2026 Includes breakfast, lunch and evening reception. $125.00
Day 1 Lunch Guest Ticket $50.00
April 23rd Evening Reception Guest Ticket $25.00
One Day Only: Friday, April 24, 2026 Includes breakfast and Awards Lunch; does not include Thursday evening reception. $210.00
Student One Day Only: Friday, April 24, 2026 Includes breakfast and Awards Lunch; does not include Thursday evening reception. $105.00

Substitutions/Cancellations:

If you cannot attend the Forum you can send a substitute, even at the last minute. If you must cancel entirely, your request for a refund – minus a $60 processing fee – must be made in writing no later than April 1, 2026. Refunds will be processed within two weeks of the request.

Cancellations made after April 1st are not eligible for a refund.
