Day One of the Forum will focus on community health improvement strategies to address social determinants of health and advance the vital conditions for health and well-being in your communities.  \nRegardless of the type of organization you serve\, board members must consider their governing style\, and ensure that they are performing at the highest level. The consequences of mediocre governance can have significant impacts on the performance of the organization\, as well as the health of those who live in the communities you serve. SUMMARY:2024 Appalachian Health Leadership Forum
DESCRIPTION:Join us April 26 & 27\, 2024 to learn about Strengthening Prosperity & Community Throughout Appalachia!\nStonewall Resort & Conference Center\, Roanoke\, WV\nRegistration for this event has closed as we have reached capacity. No on-site registration will available for this event.\nWhether you serve on the local subsidiary board of a health system\, a board of a local health department\, or the board of a community health center\, critical access hospital or rural hospital\, the Forum is for you. Health care and public health boards and their leadership are beginning to work collaboratively with other sectors in their communities to address social determinants of health to create a culture of health that will unleash economic prosperity in their communities. This is the next step beyond the “population” health work\, i.e. that work focused on addressing social determinants of health in your patients\, you are currently doing. The Forum will provide you the tools and knowledge that you need to impact the health of all who live in the communities you serve. \nThe Appalachian Health Leadership Forum will focus on two areas… \nWe know that the community conditions where we live\, work and go to school have a great impact on our health long before we ever see a health care provider. Thus\, the Forum will explore strategies related to community health improvement planning to strengthen your capacity to work with business\, government\, religious\, education\, and other leaders to develop a more coordinated\, strategic approach to identifying community priorities and taking coordinated action to achieve local economic and health priorities. Day Two will feature state and national experts on board governance best practices. \nAgenda:\nDownload the 2024 Agenda \n \n  \nRegistration Information:\nRegistration for this event has closed as we have reached capacity. SUMMARY:Take Your Best Shot: Informational Session
DESCRIPTION:Register:\nWIN is hosting an informational session via Zoom as an overview and opportunity to ask questions on the “Take your Best Shot: WV Adolescent Immunization Project.” \nPlease complete the SurveyMonkey Registration form to attend the session. \n A Zoom link will be sent to the email address provided in the registration form within one business day of registration.   \nNOTE: CEUS are not available for this informational session.  \nOverview\nThe “Take Your Best Shot: WV Adolescent Immunization Project” (TYBS) was developed to address the problems associated with low vaccination rates among adolescents throughout West Virginia. TYBS focuses on the use of evidence-based strategies by health care providers\, local health departments\, and other partners throughout the state. Thus\, TYBS projects are focused on improving HPV\, meningococcal B\, influenza and COVID-19 vaccination rates among adolescents 11-21 years of age\, in accordance with the Advisory Committee on Immunization Practices’ recommendations. \nPay for Success Project\nThe Take Your Best Shot: WV Adolescent Immunization Project is now a Pay for Success (PFS) project\, linking payment to performance in increasing immunization rates for influenza\, HPV\, COVID-19 and Men B.  The PFS strategy will ensure that high-quality\, effective immunization services are resulting in increased adolescent vaccination rates. \nTraditionally\, grants have been based on the volume of services delivered or short-term outputs. An outcome\, however\, is a longer-term change. In a PFS contract\, the grantor agrees to provide funding if and when the services delivered achieve a pre-agreed-upon outcome\, in this case improved adolescent immunization rates. WIN intends that the PFS strategy will identify new\, innovative strategies that result in improved adolescent vaccination rates in West Virginia for the targeted vaccine-preventable diseases. \nEligible Applicants and Lead Agency Selection\nEligible lead agency applicants include public and private entities\, such as local health departments\, community health centers\, hospitals\, school-based health clinics\, private clinics or other organizations. *Lead agencies currently funded under the 2022-2023 Take Your Best Shot project are not eligible for this PFS project due to overlapping project periods. \nUp to 6 lead agencies will be selected to take part in the PFS project. Lead agencies will enroll in the project by submitting a workplan that describes the strategies the applicant plans to implement\, stating how it will improve HPV\, influenza\, COVID-19 and Men B vaccination rates. Lead agencies are expected to include evidence-based and/or novel strategies in their workplan that focus on improving HPV\, meningococcal B\, COVID-19 and influenza vaccination rates among adolescents ages 11-21 years of age. The Take Your Best Shot review committee\, an ad hoc subcommittee of the WIN Steering Committee\, will then review applications and select lead agencies to be included in the project based on the (1) anticipated impact and (2) the sustainability and replicability of the strategies described in their application. \nFor more information and a link to the application click here.

SUMMARY:2022 Appalachian Health Leadership Forum
DESCRIPTION:  \n\nJoin us May 6th and 7th\, 2022 for the Appalachian Health Leadership Forum: The Resort at Glade Springs\, Daniels\, WV\nInvesting in the Health of Appalachia\nWhether you serve on the local subsidiary board of a health system\, a board of a local health department\, or the board of a community health center\, critical access hospital or rural hospital\, the Forum is for you. Health care and public health boards and their leadership are beginning to work collaboratively with other sectors in their communities to address social determinants of health to create a culture of health that will unleash economic prosperity in their communities. \nThis is the next step beyond the “population” health work\, i.e. that work focused on addressing social determinants of health in your patients\, you are currently doing. The Forum will provide you the tools and knowledge that you need to impact the health of all who live in the communities you serve. \nThe Appalachian Health Leadership Forum will focus on two areas. First\, regardless of the type of organization you serve\, board members must consider their governing style\, and ensure that they are performing at the highest level. The consequences of mediocre governance can have significant impacts on the performance of the organization\, as well as the health of those who live in the communities you serve. \nSecondly\, we know that the community conditions where we live\, work and go to school have a great impact on our health long before we ever see a health care provider. \n\n\n\n\n  \n\n\n\n\n\nClick here for the Forum Agenda.\n\n\n\n\n\nClick here to register for the\n2022 Appalachian Health Leadership Forum.\n\n\n\n\n\nBook your room reservations now as the Resort has sold out for past Forums. For reservations call (855) 516-1090 or register online at gladesprings.com. Room blocks are available until April 1st by referencing The Center for Rural Health Development.\n\n\n\n\n\n\n\n\n\nMark your calendar for the 2023 Appalachian Health Leadership Forum\, April 28th and 29th at Stonewall Resort & Conference Center.
